Qualifying for a Home Visit

Patients eligible for house calls are home limited and require the assistance of at least one other person to get to medical appointments. Our patients share a difficulty in seeking medical care in a traditional office based practice. We visit our patients in:

• private homes
• foster and group homes
• assisted living facilities
• select nursing homes
• any legal residences
